01What is the Training Effectiveness Survey?
The Training Effectiveness Survey is a ready-to-launch questionnaire that measures whether a training program actually improved knowledge, built skills and changed behavior on the job. Its 22 questions ask participants to rate statements such as whether the training improved their knowledge of the subject, whether the content was directly relevant to their role, and whether they have applied what they learned to day-to-day work. It also checks whether the depth of content was appropriate and whether managers support applying the learning. Run it shortly after a session, and again weeks later, to see whether training delivers real value rather than just satisfaction in the room.

04How to use this free template
Copy or download all 22 questions and load them into your preferred survey tool, such as Google Forms, Typeform or Microsoft Forms. No signup is required to use the template. Keep responses anonymous so participants rate the program honestly, including its weaker parts. Send it soon after the session for reaction and learning, then send the application and impact questions again a few weeks later once people have had time to use the material. Review results by theme and by program, and use the findings to refine content and reinforce manager support.
